You can request an appointment by calling or texting us at 870-881-9311. We do not require a referrals for most appointments.

What to Bring to Your Appointment

For your appointment, please bring a picture ID and your insurance card(s). We will also need a list of your current medications, including over the counter medications. You should also bring any medical records you can provide, including any test results from other clinics.

How do I get a copy of my medical records?

One option for the electronic format is unsecured email. The total file size must be less than 8MB. Only text documents are eligible for email. We do not keep pictures at our office. If you’re requesting images please contact the facility where you had your procedure done.

FMLA/Disability Requests


FMLA and Disability paperwork are common forms we fill out after surgery.

Your employer will provide you with the necessary paperwork. Please bring this to our office as early as possible prior to your surgery. The estimated turn around time is a minimum of 2 weeks from the time we receive the paperwork.
